What It Does?
Cytotec is used for treatment of gastric ulcers during concurrent treatment with anti-inflammatory drugs (salicylates and other non-steroidal anti-inflammatory drugs). It can also be prescribed for prophylaxis of peptic ulcers in patients with increased susceptibility to developing NSAID-related peptic ulcers.
How It Works?
Cytotec active ingredient, misoprostol, protects the gastric mucosa by increasing the production of mucus in the stomach and the secretion of bicarbonate in the duodenum. Cytotec also inhibits gastric acid secretion and reduce nocturnal gastric acid secretion.
Misoprostol is a synthetic prostaglandin E1 analogue. Misoprostol inhibits both basal and stimulated gastric acid secretion (histamine, pentagastrin, food and coffee) and decrease nocturnal gastric acid secretion. Misoprostols antisecretory effect is mediated by a direct effect on the parietal cells. Misoprostol has no or negligible effect on gastrin secretion.
Prophylactic treatment with misoprostol administered at 0.2mg provides protection against acetylsalicylin induced lesions in the gastric mucosa. Mucosa damage induced by necrotizing agents, reduces the treatment with misoprostol. A gastro-protective effect in humans has been observed, which manifests itself in reduced gastric and fecal blood loss after aspirin when misoprostol is given in doses of 25-50 micrograms doses only exerts a slight anti-secretory effect. The mechanism behind misoprostol mucosa protection is multifactorial. Misoprostol stimulates normal physiological mechanisms of gastro duodenal mucosa channel. Among other things, increased production of protective gastric mucus (both adherent layer of gastric juice) and the secretion of bicarbonate in the duodenum.
How It Is Taken?
Adults should take Cytotec 0.2mg 2-4 times daily. Cytotec should be taken immediately after meals, and final dose at bedtime. Cytotec and the anti-inflammatory agent should if possible be taken at the same time.
It does not require dosage adjustment in the elderly. The dose should be reduced in patients with impaired renal function.
Is It For Me?
Do not use Cytotec:If you are allergic to misoprostol, other prostaglandins, or any other ingredients of this medicine (hypromellose, microcrystalline cellulose, sodium starch, hydrogenated castor oil).
If you are pregnant, if pregnancy is not excluded or if you are trying to become pregnant because it can cause miscarriage. Women who have not reached the menopause should use adequate contraception before treatment with Cytotec starts.
Any Drug Interactions or Incompatibilities?
Co-administration of NSAIDs and misoprostol in rare cases cause elevated transaminase levels and peripheral edema.
Cytotec is metabolised mainly via the systems for oxidation of fatty acids and have not shown any negative effect on the hepatic microsomal oxidase P450 enzyme system. In specific studies, no clinically significant pharmacokinetic interaction was demonstrated with antipyrine or diazepam. A moderate increase in propranolol concentrations have been observed at multiple doses of misoprostol.
Interaction studies with misoprostol and several NSAIDs showed no clinically significant effect on the kinetics of ibuprofen, diclofenac, piroxicam, aspirin, naproxen or indomethacin.
Magnesium-containing antacids should be avoided during treatment with misoprostol as this can exacerbate misoprostol-induced diarrhea.
